Understanding breast augmentation (mammoplasty) with lift (mastopexy) surgery

Breast augmentation mammoplasty, and breast lift or mastopexy, are both stand-alone procedures that can be performed in isolation. However, some patients desire the effects of both surgeries. In these cases, both surgeries can be performed in one single procedure, known as a combined surgery.

Usually, a combined augmentation mammoplasty and mastopexy procedure may be performed in cases where the breasts have a moderate or severe degree of laxity and loss of volume. For these patients, implants alone may not be enough to raise the position of the breasts, and a breast lift (mastopexy) alone may not provide the desired volume. By combining both procedures, the patient can ameliorate a low breast position and loss of volume simultaneously.

The goal in using both techniques is to customise to the patient’s needs. In the first consultation, we will assess your needs and discuss your goals and expectations, discussing the various options for your procedure. For example, we will discuss your desired size, whether you want implants or fat grafting, and potential incision methods. We will also outline the potential risks and complications, along with the recovery instructions, to ensure you are well informed about your options and chosen procedure.

Suitable candidates for breast augmentation with lift (mammoplasty with mastopexy)

Suitable candidates are those who have experienced a loss of breast volume, along with changes in breast shape and position. These changes are often due to weight fluctuations, pregnancy and breastfeeding, or ageing. Patients may have a moderate to severe degree of breast laxity, in addition to a desire for a larger breast size – often, this means that they may not see their desired results from an augmentation mammoplasty or mastopexy alone.

Patients who wish to raise the position of their breasts but have no desire to change their breast size may be more suitable for a breast lift (mastopexy) only. Likewise, those who desire a larger breast size and have only mild laxity may be suitable for an augmentation mammoplasty alone. Your surgeon will determine whether your natural anatomy and cosmetic desires are such that combining the two surgeries would be appropriate.

In addition, your suitability for surgery depends on your medical history, smoking status, and understanding of the risks and recovery involved. During an initial consultation, your surgeon will walk you through your options and the most appropriate surgery methods in your unique case. What to expect on the day of the procedure

On the surgery date, patients will need to undergo the appropriate pre-surgery preparations, including a review of the surgical plan to ensure you and your surgeon are on the same page. This also gives you the opportunity to ask any last-minute questions or raise any concerns. The procedure is performed with a general anaesthesia.

The surgeon proceeds with your surgery according to the specifications outlined in your treatment plan. The exact techniques will depend on your needs. For example, the incision methods, implant size, and whether fat grafting is included all depend on your circumstances and personalised procedure. In the case that implants are used, the surgeon places the implants either behind or beneath the chest muscle. He also reshapes the breast tissue to help treat concerns about laxity, being mindful of proportions and symmetry. In the case of fat grafting, the surgeon performs liposuction on a different area of the body to harvest fat tissue, then transfers the processed fat tissue to the breasts.

Following surgery, the recovery period begins, and you will be given aftercare instructions. Since both surgeries are performed at once – the reshaping of the breast with mastopexy surgery, and the application of breast implants or fat grafting with augmentation mammoplasty – patients undergo a single recovery period.

Aftercare and Recovery

Caring for your body properly post-surgery is important to support your recovery and reduce risks and complications where possible. We will give you instructions to guide you, and it’s important to follow these instructions and let us know if any concerns arise.

Patients need to take time away from work to recover, as well as time away from their usual activities and exercise routines. It’s important to support your healing breasts by wearing a surgical bra and avoiding putting any pressure on your breasts. At your regular follow-up appointments, we will monitor your healing and assess the progress of your recovery to ensure you are getting the care you need.

Other instructions may include keeping your incisions clean, avoiding strenuous activities or lifting, and avoiding smoking throughout recovery. You will be encouraged to resume light walking, as this can promote blood flow and healing, but ensure not to overdo it. Sleeping elevated and on your back can also help support recovery.

Risks and complications

In all cases, surgery involves risks and complications, and you will need to discuss these to ensure you understand and are fully informed. Risks can include:

  • Bleeding or haematoma
